Important information for your first visit:
1) Please arrive 10 – 15 minutes before your first scheduled appointment to register at the front desk.
2) Bring your Doctor’s prescription, Insurance card, X-rays, MRI, and Surgical reports.
3) Expect to be at our clinic for approximately 60-75 minutes, for your first visit.
4) Bring or wear comfortable clothing such as a tank top, shorts, sweats and tennis shoes.
