Important information for your first visit
1) Please arrive 10 -15 minutes before your first scheduled appointment to register at the front desk.
2) Bring your Doctor’s prescription, Insurance card, X-rays, MRI, or Surgical reports.
3) Expect to be at our clinic for approximately 60 – 75 minutes, your 1st visit.
4) Bring or wear comfortable clothing such as a tank top, shorts and tennis shoes.

We are happy to verify your benefits regarding physical therapy services
prior to your first visit. Please note that contracted providers may be added or deleted at any time, given unexpected contract changes. Please contact our office to verify that we are currently a provider of physical therapy services for your insurance company.
